Paper DBE Application Process
Documentation Overview
Businesses opting to apply for DBE certification using the paper process must submit both of the following documents:
- Uniform Certification Application (UCA)
- DEB / ACDBE Personal Net Worth (PNW) Statement
The documents must be fully completed and include all required supporting documentation identified on page 17 of the certification application.

Maintaining Your DBE Certification
DBEs / ACDBEs must do the following things to maintain their certification.
Submit New DOE form every year
DBEs / ACDBEs must provide every year on the anniversary of their original certification, a new Declaration of Eligibility (DOE) along with the signed federal business tax returns (including tax returns of affiliate firms).
Submit the DBE or ACDBE Declaration of Eligibility (DOE) online using CCRTA’s online portal or download and submit the form and supporting documentation requirements to the DBE Office at:

Keep Us Informed of Your Business Status
DBEs / ACDBEs must inform CCRTA in writing of any change in circumstances affecting their ability to meet size, disadvantaged status, ownership, or control requirements or any material change in the information provided in the original application.
DBEs / ACDBEs must notify CCRTA of a material change in their circumstances that affects their continued eligibility within 30 days of the occurrence, explain the change fully, and include a duly executed DOE with the notice. The DBEs’ non-compliance is a §26.109(c) failure to cooperate.
